Danielle Schwartz

Danielle Schwartz, BSN RN, currently serves as a Registered Nurse Case Manager at BAYADA Home Health Care since July 2024, following a similar role at Panoramic Health from October 2022 to May 2024, where responsibilities included managing high-risk CKD/ESRD patients and collaborating with interdisciplinary teams.
