Jake Butler

Jake Butler is an experienced product leader with a strong background in healthcare technology and user experience. Currently serving as Head of Product - Patient Experience at FORM, a virtual obesity medicine clinic since March 2021, Jake has previously held significant roles at Rally Health as Director of Product Strategy and Senior Product Manager for the Rally Recover program, which enhances post-acute surgical recovery for patients.
