Comprising healthcare professionals such as the Chief Clinical Officer, Medical Director, and support staff, the team ensures seamless coordination of hospice services, evaluates clinical protocols for quality assurance, and provides emotional and spiritual support through the Chaplain, fostering a holistic approach to supportive care in the community.
