Healthcare Team

The Healthcare Team at SeMHRI collaborates to deliver comprehensive and compassionate primary and preventive health care. Led by the Coordinated Care Manager and Health Center Manager, the team, including family nurse practitioners, a pharmacist, a licensed clinical social worker, a registered nurse, and a Ryan White Outreach Worker, ensures patient-centered care that addresses both medical and social needs in the community.
