Triniti Home Health & Hospice is jointly owned by Guardian Angels Senior Services of Elk River and Cassia, an Augustana/Elim affiliation. Triniti Home Health & Hospice employs a community of health care professionals consisting of nurses, social workers, therapists, chaplains, music therapists, massage therapists, volunteers, and more. We serve people in Anoka, Carver, Dakota, Hennepin, Isanti, Mille Lacs, Ramsey, Scott, Sherburne, Washington, and Wright counties, as well as eastern Stearns, Benton, McLeod and Meeker counties in Minnesota.
