Nicole Hawk

Nicole Hawk has been working at Aging Solutions since September 2018 as the Director of Strategic CARE Planning. Prior to that, she worked as a Physician Assistant at Medexpress Urgent Care for 8 years. Nicole holds a Master's degree in Health Services/Allied Health/Health Sciences from Drexel University College of Nursing & Health Professions Division of Undergraduate Nursing. Her role at Aging Solutions involves administering and supervising the CARE Method, developing care plans, supervising information flow, interviewing clients and families, and collaborating with team members and specialists. Nicole also works closely with the Data Integration Coordinator to produce critical data for the program's outcomes.
