Nefisa Hassen

Nefisa Hassen is a dedicated healthcare professional with extensive experience in care coordination and community health navigation. Currently serving as a Care Coordinator at NeueHealth since May 2022, Nefisa previously held the position of Community Health Navigator at Kaiser Permanente, managing over 40 cases and providing essential support and education to patients. Nefisa also worked as a Family Strengthening Advocate and Case Manager at IRCO, where knowledge of cultural diversity was applied in case management for domestic violence victims. Additional roles include serving as an Interpreter and Election Clerk. Nefisa Hassen is pursuing a Master's degree in Data Analytics at Western Governors University, building upon a Bachelor of Science in Public Health obtained from Portland State University.
