Dave A. Chokshi

Commissioner, Department of Health and Mental Hygiene at City of New York

Dave A. Chokshi is the Commissioner of the Department of Health and Mental Hygiene at the City of New York. Dave was previously the Chief Population Health Officer at NYC Health + Hospitals from October 2014 to May 2020. In this role, they led a team of over 700 staff across Population Health, Community Care, and the Accountable Care Organization. Dave was also responsible for programs in primary care transformation, healthcare analytics, innovative care models, chronic disease management and prevention, social determinants of health, care management, value-based care, and research and evaluation.

Prior to their role at NYC Health + Hospitals, Dave A. Chokshi was the Director of Population Health Improvement (Network Integration) at NYU Langone Medical Center from December 2013 to October 2014. Dave was also an Attending Physician at Bellevue Hospital Center from 2013 to 2014.

From September 2013 to September 2014, Dave A. Chokshi served as a Senior Advisor to the Commissioner at the New York State Department of Health. Dave was also a Visiting Fellow at the Patient-Centered Outcomes Research Institute (PCORI) from September 2013 to December 2013.

Dave A. Chokshi graduated from Baton Rouge Magnet High School and then went on to Duke University where they received their BA in Chemistry, Public Policy, Biology. Dave then attended the University of Pennsylvania School of Medicine where they obtained their MD. After that, they completed their MSc in Global Public Health, Comparative Health Policy from the University of Oxford.
