Patricia Mullaney is an experienced healthcare professional with a strong background in business development and patient care transition. Currently serving as Care Transition Liaison at Community Nurse Home Care since July 2018, Patricia enhances patient referrals and admissions through exceptional relationship development. Previously, Patricia held the position of Hospital Liaison at Golden Living Centers from June 2013 to July 2018, focusing on business development in hospital accounts.
