Kimberly Moreland

Kimberly Moreland is an experienced social worker and case manager with a career spanning over a decade in healthcare and social services. Currently serving at Connecticut Community Care, Inc. since March 2019, Kimberly focuses on case management and support for patients and families, including discharge planning and linking individuals to community services.
