Rachel Risler

Rachel Risler, LCSW, is an experienced clinical social worker specializing in aging and healthcare. Currently serving as a Care Manager at the North Carolina Community Health Center Association, Rachel provides expert care management for Medicaid enrollees. Previous roles include delivering psychotherapy services to long-term care populations at Eventus WholeHealth and providing in-home counseling and care management to seniors at Care Navigators. Rachel has extensive experience in hospice care, having worked as a social worker at JourneyCare. Additionally, Rachel has held leadership positions as Director of Social Services at both Whitehall of Deerfield and Wealshire, focusing on psychosocial assessments and staff education. Rachel’s educational background includes a Master of Social Work from the University of Michigan and a Bachelor of Arts in Government from Smith College.
