Graciela Diver

Graciela Diver is a Family Nurse Practitioner with extensive experience in various healthcare settings. Since June 2022, Graciela has been serving at Healthcare Associates of Texas, providing comprehensive care through child well visits, physicals, and Medicare well visits. Prior to this role, Graciela worked at Baylor Scott & White Health, Premier Independent Physicians Group, and Mi Doctor clinics, conducting patient assessments and formulating care plans. Graciela's career began at Medical City Healthcare, where responsibilities included patient admissions in the endoscopy lab and step-down ICU. Residing in patient homes, Graciela also provided long-term care as a Home Health Registered Nurse. Graciela holds a Master’s degree in Family Practice Nursing from Texas Tech University Health Sciences Center, a Bachelor's degree in Registered Nursing from Western Governors University, and an Associate of Arts and Sciences in Registered Nursing from Brookhaven Community College.
