Monika Fatu

Monika Fatu, LCSW, is an experienced social worker with a robust background in case management and community care navigation. Monika has been serving as a Case Manager II and Community Care Navigator at Hoag Hospital since August 2017. Prior to this role, Monika worked at People Assisting The Homeless (PATH) from August 2015 to August 2017 as a social worker. Monika holds a Master of Social Work (MSW) degree from California State University, Fullerton, obtained between 2013 and 2015, as well as a Bachelor of Social Work (BSW) degree from La Sierra University, completed from 2010 to 2012.
