Nichole Hoffman

Nichole Hoffman is an experienced healthcare professional with a comprehensive background in patient care coordination and admissions management. Currently serving as a Patient Care Coordinator at Residential Home Health and Residential Hospice since June 2017, as well as at Hometown Home Health and Hospice since 2017, Nichole brings expertise in overseeing patient needs. Previously, Nichole held the position of Director of Admissions at PineRiver Healthcare from July 2014 to May 2017, where responsibilities included conducting assessments for long-term care placements and managing external marketing efforts. Nichole's earlier experience includes roles in sales and marketing at 95.3 WCFX and serving as Executive Director for the United Way of Isabella County for a decade, along with a position as Activity Director at Schnepp Health Care Center from 1995 to 2001. Nichole earned a Bachelor of Arts degree in Commercial Recreation from Central Michigan University between 1991 and 1995.
