Sandeep Palakodeti

Sandeep Palakodeti is the current Chief Medical Officer at Hopscotch Health. Prior to this, Palakodeti served as the Chief Medical Officer, Population Health for University Hospitals from June 2020 to November 2021. In this role, Palakodeti provided clinical and executive leadership for the Population Health division of a $4B+ health system and served on the management team of one of the largest ACOs in the country, with 550k+ members, and >100k Medicare and MA lives.

As Chief Medical Officer, Population Health, some of Palakodeti's accomplishments include:

- Clinical liaison and medical director for value-based arrangements with all payors across all service lines

- Responsible for transformation activities across quality improvement, utilization, STARs/HEDIS performance, MA/MSSP programming, transitions of care, high-risk care management, deployment of predictive analytics, and SME for clinical and payor-related issues across the system

- Part of the team that delivered a Top 5 performance nationally in the MSSP program, with a 100% quality score and well-managed utilization/cost of care

- Core institutional leader managing self-insured employee plan and wellness activities

- Served as Co-Founder and Clinical Lead for a new system-wide service line, the UH Outpatient Infusion and Speciality Clinic, a novel new model serving chronically ill patients needing injectables and servicing high-risk members needing evaluation and management prior to further decompensation

Sandeep Palakodeti completed a fellowship in healthcare innovation and entrepreneurship at Harvard Medical School. Sandeep then completed an internal medicine residency at Kaiser Permanente San Francisco. Sandeep received their MD and MPH from Wright State University Boonshoft School of Medicine. Sandeep received their bachelor's degree in premedicine from the University of Dayton. Sandeep is certified by the Institute for Healthcare Improvement in healthcare leadership, patient safety, and quality improvement.
