Amy Fernandes-Major LCSW is an experienced social worker with a robust background in various healthcare settings. Currently serving as an MLTSS Care Specialist III at Horizon NJ Health since August 2014, Amy has previously held medical social work positions at St. Joseph's Regional Medical Center, focusing on discharge planning and crisis intervention.
