Carol Ann Jackson

Carol Ann Jackson has extensive experience in nursing and care coordination, currently serving as an Intake Care Coordinator at Long Term Solutions since May 2015, where responsibilities include evaluating assessments and developing care plans for long-term care clients. Previously, Carol Ann worked at the VNA of RI from November 2005 to March 2015 as a Palliative/Hospice RN, delivering nursing services to patients with medical needs and providing support for end-of-life care. Additionally, Carol Ann held positions as an RN at Roger Williams Medical Center from June 1999 to September 2005 and as Director of the Alzheimer’s Unit at Epoch at Blackstone Blvd. from January 2005 to July 2005. Carol Ann earned a Bachelor of Science in Nursing (BSN) from Rhode Island College between 1994 and 1997 and graduated with a diploma from Woonsocket High School in 1976.
