Donna Crane

Donna Crane is an accomplished healthcare professional with extensive experience in health home operations and clinical program coordination. Currently serving as the Manager of Health Home Operations at Monroe Plan for Medical Care since September 2005, Donna has a robust background in managing health programs, having previously held the role of Clinical Program Coordinator at the same organization. Prior experience includes positions as Manager of Southern Tier Health Home Operations and Clinical Program Coordinator at Monroe Plan for Medical Care, as well as case management at United Health Home Care and office management at Cardiology Associates. Donna holds a Master of Science in Nursing (MSN) with an Executive Track from Chamberlain University, a Bachelor of Science in Nursing (BSN) from Chamberlain College of Nursing, and an Associate of Arts and Sciences in Nursing from Niagara County Community College.
