Jane Jackson

Jane Jackson has an extensive background in healthcare management and nursing, spanning over three decades. Since January 2009, Jane has been with Monroe Plan for Medical Care, currently serving as VBP Clinical Practice Performance Support and as an RN Case Manager. At YourCare Health Plan, a Monroe Plan Company, Jane has held several positions since its inception in August 2015, including Health Home Quality Program Manager and HEDIS/Health Plan Quality, and has played a key role in developing the Health Home Quality Program. Prior experience includes roles at Buffalo Cardiology and Pulmonary Associates as an RN in the Coumadin Clinic, Kaleida Health as RN Cardiac Research Coordinator, and at UniVera HealthCare/Health Care Plan, demonstrating a solid foundation in clinical care and quality improvement in healthcare settings.
