Tonianne Costa

Tonianne Costa is a seasoned Registered Nurse with extensive experience in both hospital and population health settings. Since November 2019, Tonianne has served as the Lead Population Health Nurse at Palm Beach Accountable Care Organization, focusing on value-based healthcare to enhance early disease detection and prevention through actionable reports. Prior nursing roles include positions in the Mother Baby Unit and Postpartum at Winchester Hospital from February 2016 to October 2019, and at Massachusetts General Hospital in the Thoracic Surgery Unit from September 2015 to February 2016. Tonianne began the nursing career as a Certified Nursing Assistant in the Medical Surgical and Telemetry Unit at Winchester Hospital from January 2014 to September 2015. Tonianne holds a Bachelor of Science in Nursing from Salem State University, earned in 2015.
