Leslie Nguyen

Leslie Nguyen is a dedicated healthcare professional with experience in medical management and care coordination. Currently serving as a Medical Management Personal Care Coordinator at Santa Clara Family Health Plan since August 2020, Leslie excels in contacting patients for health risk assessments and collaborating with case managers. Leslie has also been a Care Provider at Premier Health Services, Inc since June 2016, focusing on daily routines and life skills development for clients with autism. Previous experience includes an internship as a GoodHealth Intern at Goodwill of Silicon Valley, where Leslie created health lesson plans for high school students with autism, and roles as a Teaching Assistant at Kumon and a Behavior Interventionist at Bay Area Behavioral Connection, where organizing data and maintaining safe environments were key responsibilities.
