Kate Wagner

Kate Wagner is currently the Executive Director at Prime Care Coordination since March 2020. Prior to this role, Kate served as Vice President of Operations at CDS Life Transitions from April 2012 to March 2020. Kate holds a Master of Science in Health/Health Care Administration/Management from Roberts Wesleyan University, obtained from October 2018 to May 2020. Kate also holds a Bachelor of Science in Nursing from Roberts Wesleyan University, received from September 2006 to December 2011.
