Polly Street

Polly Street is an experienced healthcare professional with a focus on strategic partnerships and population health management. Currently serving as the Director of Strategic Partnerships at Optima Health and Sentara Healthcare since October 2013, Polly has previously held significant roles including Senior Director of Population Health Care Management at Evolent Health and Senior Director of Population Health at Healthways. Polly's career spans several leadership positions in healthcare organizations, including Inova Loudoun Hospital, the University of Rochester, RCIPA, Genesee Hospital, Upstate Health System, and St. Mary's Hospital. Polly holds a Bachelor of Science degree from the University at Buffalo and a Master of Science degree from the University of Cincinnati.
