Connie Hollenbach

Connie Hollenbach has extensive experience in healthcare and social work, currently serving as a Community Liaison at Trinity Health At Home since October 2015. Prior to this role, Connie worked at Extendicare Health Services, Inc. from April 2007 to September 2015, where responsibilities included serving as a Referral Manager and Social Worker at Statesman Health and Rehabilitation.
