Rachel Hughes

Rachel Hughes, MHA, serves as the Manager of Population Health Operations at Vytalize Health since October 2022, contributing to a provider enablement platform that enhances patient-care relationships through data-driven, personalized care. Rachel's previous roles include Senior Analyst and Analyst in Population Health Operations at VillageMD, and Care Manager/Quality Manager at Twelve Oaks Pediatrics. Additionally, Rachel has experience as an administrative intern at Trinity Health and Green Tree Pediatrics, where project management and quality reporting were key responsibilities. Educational accomplishments include a Master’s degree in Health Administration from Oklahoma State University Center for Health Sciences and a Bachelor’s degree in Health Care Administration from Madonna University, complemented by an Associate’s degree from Schoolcraft College.
